Who's eligible for the Fortress Plan?

Applicant Requirement

  • All applicants must live in Texas.
  • Every plan requires at least one adult (No child-only policies)
  • Primary applicant must be 18 or older.
  • Dependents are covered up to age 25.
  • Newborns are covered from birth if enrolled within 31 days.
  • Disabled adult dependents over age 25 are eligible with documentation.

Identity & Citizenship

All applicants must have a valid SSN or ITIN and must be U.S. Citizens or Lawful Permanent Residents.


Tobacco Use

Applicants may not use tobacco products more than 4 times per month.


Medical History

Applicants with a history of any condition below are not eligible. This applies to every household member on the plan — not just the primary applicant.

Addison's Disease Hepatitis (any type) Parkinson's Disease
Cancer (any type) HIV Pernicious Anemia
Crohn's Disease Kidney/Renal Failure Psoriasis
Cirrhosis Lou Gehrig's Disease (ALS) Reactive Arthritis
Congestive Heart Failure Lupus Rheumatoid Arthritis
Dementia Lyme Disease Scleroderma
Dermatomyositis Mitral Valve Prolapse Sjögren Syndrome
Diabetes (Type 1 or Type 2) Multiple Sclerosis Stroke
Epilepsy Muscular Dystrophy Ulcerative Colitis
Fibromyalgia Myasthenia Gravis
Graves' Disease Heart Attack (MI)
Hashimoto Thyroiditis

"History" means ever having sought, been recommended, or received medical advice, diagnosis, care, or treatment for any condition listed above.

Pre-Existing Conditions

Enrolling with a pre-existing condition doesn't automatically disqualify you — but coverage for that condition is limited in your first year.


Any condition for which you sought, received, or were recommended care in the 12 months before your start date will not be covered during your first 12 months of coverage.


After the waiting period ends, pre-existing conditions are covered like any other eligible condition under your plan.
