What are the policy maximums?

There are no policy maximums for the Specified Disease or Accident Policies. The optional Wellness Policy pays a lump-sum, "fixed indemnity" amount for specified, relatively low-dollar, predictable preventative services. Some select services such as physical therapy may have treatment or dollar limits applied. Please see your policy for these specific limits.
